Nealy Pierce is partnering with a respected nonprofit 700+ bed health system to recruit an experienced Case Management Director (RN).

The incoming Director will step into a well-established department built by a highly respected leader who will be retiring. With over 60 team members and 3 managers already in place, the new leader can focus on advancing performance rather than building the department from the ground up.

Areas of Responsibility

• System-wide Case Management and Utilization Review
• Discharge planning and length-of-stay performance
• Palliative Care coordination and Social Work services
• Complex patient placement and bed-availability challenges
• Relationships with skilled nursing, rehabilitation and home health partners
• Departmental analytics and reporting

Leadership Structure

The Director reports to the Vice President of Medical Affairs and leads:

• Two site-based Case Management Managers
• One Utilization Review Manager
• Approximately 65 Care Coordination professionals across two campuses

Qualifications

• RN Active License State of Georgia
• Large acute care Case Management or Care Coordination leadership experience
• Five to seven years of acute care experience
• Three or more years in a Case Management / Care Coordination leadership capacity
• Master’s required; BSN required with a non-nursing master’s
• CCM certification strongly preferred
