## Description

**Nealy Pierce** has been exclusively engaged by a respected, faith-based nonprofit health system in Georgia to recruit a **Director of Care Coordination (RN).**


This highly visible leadership opportunity includes system-wide oversight of:





- Case Management and Utilization Review

- Discharge planning and length-of-stay performance

- Palliative Care coordination and Social Work services

- Approximately 65 team members across two hospital campuses

- Two site-based Case Management Managers and a Utilization Review Manager


The organization is seeking a collaborative, data-driven leader who can manage a large department, strengthen physician partnerships and improve patient progression across the continuum of care.

- Report directly to the Vice President of Medical Affairs

- Lead an established 65-person department

### Requirements





- RN required

- Case Management or Care Coordination leadership experience within a large, complex hospital or health system

- Five to seven years of acute care hospital experience

- At least three years of leadership experience within Case Management or Care Coordination

- Master’s degree required (if not in Nursing, must hold a BSN)

- CCM certification strongly preferred
