A respected nonprofit 700+ bed health system has exclusively engaged Nealy Pierce to recruit a Director of Clinical Care Coordination. The Director will lead approximately 65 professionals across two hospitals and report directly to the Vice President of Medical Affairs.

The Opportunity

This leader will guide:

• Case Management and Utilization Review across both campuses
• Discharge planning and length-of-stay improvement
• Palliative Care coordination and Social Work services
• Complex patient placements and bed-availability escalations
• Partnerships with post-acute providers
• Care Coordination analytics and performance reporting

Who They Need

• Proven Case Management or Care Coordination leadership in a large acute care environment
• Five to seven years of hospital experience
• At least three years of progressive leadership
• Master’s degree required; BSN required when the master’s is outside nursing
• CCM certification preferred
• Active RN license state of Georgia
