Nealy Pierce has been exclusively retained to recruit a Director of Care Coordination (RN) for a Magnet designated, nonprofit health system in the coastal Southeast. This leader will oversee approximately 65 team members across two hospital campuses.

What You Will Own

• Case Management, Utilization Review and discharge planning
• Length-of-stay strategy and patient progression
• Palliative Care coordination and Social Work services
• Complex placement and bed-availability escalations
• Post-acute relationships with SNFs, rehabilitation facilities and home health agencies
• Departmental metrics, analytics and executive reporting

Qualifications

• Active RN license required
• Progressive Case Management or Care Coordination leadership in a large acute care hospital or multi-hospital system
• Five to seven years of acute care experience
• Three or more years in a Care Coordination leadership role
• Master’s degree required; candidates with a non-nursing master’s must possess a BSN
• CCM certification preferred
